Financial Policies In our office, you have a variety of financial options from which to choose. Payment may be made by cash, personal checks (drawn on a local bank), Master Card, or Visa. Payment in full is due at the time of service unless other advance arrangements have been made. No insurance is accepted for your first visit. The following payment options are also available to you: A 5% courtesy is discounted from statements of $2500 or more which are paid in full by cash or check prior to the first treatment appointment. Insurance payments are accepted and we will do everything possible to help you maximize your benefits. You are responsible for the difference between the actual insurance payment and the fee. If you have a secondary insurance policy, we'll provide you with the needed documentation for your claim. If your insurance company fails to pay in whole or in part for any reason within 30 days, you must pay for the remaining balance. Interest (1.5%) will be charged after 30 days.You are responsible for the fees that you incur with Frank M. Addabbo, D.D.S., PA., not your insurance company or other third party. You are also responsible for any co-payment and/or deductible, and parents/guardians are responsible for payment of a minor. If you fail to pay on your account, we reserve the right to turn your account over to a collection agency or attorney for collection. If this account is assigned to an outside collection agency, an additional fee of 50% of the total amount owed will be added. There is a $25.00 charge for checks returned for insufficient funds. Cancellation Policies You must give at least 48 hours notice if you need to cancel an appointment. You may be charged in full for the appointment - even if you do not attend - if less than 24 hours notice is given. A charge of $25 per 15 minutes may be charged if cancellation is between 24 and 48 hours before the appointment. Business days are Monday through Thursday. |
